a. Assessment · Conduct assessment on assigned patient according to the hospital protocol & by completing & recognizes changes in patient condition, while delivering nursing care. · Assesses patient’s environment for safety, cleanliness and proper functioning of equipment’s. · Assess patients/Families’ health education and discharge needs and intervenes accordingly. b. Planning · Plans her unit activity and staffing needs according to given assignment by Head Nurse, team leader /Incharge · Plans and prioritizes his/her work to provide best quality care according to patient’s need assessment. · Discuss the patient issues and plan of care with physician as required. · Plan health-teaching session for patient/families according to their learning need. c. Implementation · Provides direct care to the patients including hygiene care · Administer medication safely by following Doctors order and the rights of medication & monitors patient responses to medication therapy and reports related incidents promptly · Administer medication to the patient by maintaining. Knowledge related to medication indication and side effects · Delivers designated nursing interventions to assigned patients that are consistent with the state of medical care plan · Evaluates care given including the patient response · Perform vital sign of patient · Assess patient early warning sign · Having knowledge and use of equipment’s (Ventilator, CPAP/Bipap, infusion pump, syringe pumps, Nebulizer machine etc) · Participate in consultant round to ensure the care plan of patient · Monitor care of patient regarding level of consciousness, hemodynamic, diet, physical activity, and head to toe assessment etc. · Performs routine unit specific procedures independently and assist those procedures for which not credentialed · Involve in cardiac arrest situation for Basic life support of the patient · Demonstrates safe handling and proper use of equipment’s shift wise · Takeover of patient at bedside by having proper handing taking of patient in written · Recognizes obvious changes in patient status & seeks appropriate resources to address concern · Participates actively in staff education with the assistance of Nursing Services · Demonstrates responsibility for learning to improve self and performance · Provides safe environment to the patient, self & other health team members by following infection control policies and safety measures · Follows admission, transfer & discharge procedure · Guides staff in provision of safe nursing care and in compliance of hospital policies and procedures · Takes over of crash cart, stock medication and refrigerator item as assigned by Incharge · Takes follow-ups regarding special assignments, register maintenance and patient care · Assist doctors while performing procedures and examination · Maintains patient’s privacy and confidentiality of information and records at all times · Provide health education and discharge teaching to patient and family · Monitor & record nutritional status of all assigned patients according to the unit · Maintain a good relationship among health care team by communicating information · Responding to request building trust relationship, participate in problem solving approach · Any other related task as assigned by Supervisor, team leader, Incharge, and Manager. d. Documentation · Ensures timely and complete documentation of all the care and procedures performed as per unit protocol. · Completes accurate, concise, and legible documentation on medical records. e. Evaluation · Evaluates outcome of nursing intervention and acts accordingly to meet patient’s nursing care needs. · Keep well-groomed and observes the uniform code at all times. · Ensure that nursing practices are in line with institution policies · Maintains good working relationships with peers, health team and support services by being helpful and supportive whenever possible · Demonstrate commitment to nursing profession · Demonstrate courtesy, tact and helpful attitude to patients and families f. Responsibilities of Labor Room Staff: · Maintains proper documentation and monitoring of patients with labor pain. · Demonstrate the use of CTG machines on patients with labor pain. · Interprets CTG and takes appropriate action · Coordinated OR list with OR charge. · Ensure that pre-op assessment is completely done. · Health education for mothers · Assist in breastfeeding. · Ensure transfer out of patients safely to assigned unit. · Fill Baby registration form · Implement quality assurance programs and initiatives to monitor and improve the quality of patient care and operational processes. · Develop and implement infection control protocols and procedures to minimize the risk of healthcare-associated infections in the operation theatre.
Requirements
Minimum Qualifications: Diploma in Nursing or preferable BScN/Post RN BScN. Valid Registration with the Pakistan Nursing Council Minimum Experience: One (1) Year Internship from a reputable Institute. OR Minimum two (2) years’ experience as Registered Nurse in relevant area of specialization.
